Overview

Our Social Services Manager directs, monitors, and evaluates the social services provided in the RISE program. This position acts as a liaison between Social Workers, Re-Entry Specialists and the Forensic Program Director. This position helps supervise, organize and motivate Social Workers and Re-Entry Specialists and provides guidance and support as appropriate. In the clinical role, this position assesses, develops care plans, and treats patients from a psychosocial perspective providing related assistance in securing benefits, community and facility resources as well as developing/providing referral sources and outreach related to discharge planning specific to reducing recidivism for individuals completing the RISE program. This position conducts groups and supports a caseload of patients. This position also maintains communication links between the treatment team, families and guardians, as well as community providers.

Qualifications

Education:

  • Master's degree in Social Work or Mental Health Counseling.

Experience:

  • Three (3) years of applicable experience in a mental health setting and in the provision of psychiatric social work is required.
  • Knowledge of the DSM, the chronically mentally ill and community resources.
  • Supervisory experience preferred.

Licenses/Certifications:

  • Must have a current Licensed Clinical Social Worker.

Responsibilities

Maintenance of the social work assessment schedule.Ensures that initial and monthly psychosocial assessments, consent forms, discharge plans, and communication logs are complete and that community outreach contacts, including Re-Entry site visits are made timely.Conducts psychoeducational groups and supports a caseload of patients.Participates in the performance improvement and quality review process.Participates in employee training and provides discipline specific clinical/administrative supervision.Contributes to policy, procedures, and discipline related form updates such as visitation, psychosocial assessments, and discharge planning.Provides on-call back up to Crisis Social Worker on a rotating basis.Attends and facilitates treatment rounds.Works with the Crisis Social Worker to ensure collaborative follow up care for high acuity patients across treatment teams.Communicates concerns about program to the Forensic Program Director in a manner that is team-spirited, positive and productive.Develops and implements interventions for barriers to discharge, documents all direct and collateral contacts into the medical record, advocates and negotiates internal and external systems on behalf of the patient within the scope of clinical training and consistent with professional standards.Seeks to enhance the capacity of patients to address their own needs. Utilizes all patient contact as opportunities for therapeutic intervention.Cooperates with colleagues to serve the well-being of the patients.Participates in and contributes to decisions that affect the well-being of the patients. Seeks the advice and counsel of colleagues when it is in the best interests of patients.Attends program or company meetings and participates actively in problem solving and idea sharing within department.Applies sensitivity to cultural diversity issues, treats patients as an individual, and considers the culture of the patients when providing care and treatment.Applies knowledge about care and treatment needs to patients of different ages. Provides individualized care and treatment that is consistent with/sensitive to the age and life span developmental needs of each patient.Assists in the improvement of the program's service delivery.Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.Must be alert at all times; pay close attention to details.Must be able to work under stress on a regular or continuous basis.Perform other duties as assigned.
